肿瘤临床研究基本概念

• 总生存 overall survival，OS

  • 定义: 结局指标是死亡时间，这个死亡是任何原因导致的死亡都算进去，只关心是否死亡，不关心因为何种原因死亡。
  • 优点: 能比较方便的记录，因为患者死亡的日期确认没有困难。只要研究结果显示生存有提高，就可认为是是临床又获益。
  • 缺点: 随访的时间较长

• 无进展生存期 progression-free survival，PFS

  • 定义: 指的是疾病经过治疗后，没有进一步恶化的生存期，结局指标是 发生恶化或 死亡
  • 优点: 增加了发生恶化这一结局指标节点，随访时间短一些，对应的改善是未恶化与未死亡，可以反映临床获益。如果 PFS提高了，可以认为临床有获益。
  • 缺点: 因为增加了 发生恶化这一结局指标，我们就要问一个问题了，何为发生恶化？有没有明确的标准？相对于记录是否死亡，判断病人是否病情恶化的难度要大得多，因此这就要求对 发生恶化的标准进行明确的定义。发生恶化的定义通常涉及影像学资料（普通X线，CT扫描，MRI，PET扫描，超声）或其他方面：生化进展可以根据肿瘤标志物的增加。

• DFS 无病生存期 Disease Free Survival

  • 定义: 经过治疗后未发现肿瘤，结局指标为 疾病复发或 死亡，同样不关心死亡原因。
  • 优点: 是临床获益的重要反映，随访时间可以缩短，因为增加了疾病复发这一节点。没有复发或没有死亡可以反映临床获益
  • 缺点: 同理，因为增加了 疾病复发这一节点，我们就要问了， 何为复发？如何明确有无疾病复发？对于记录死亡，明确是否复发的难度要大得多。记录比较困难。

• DSS 疾病特异性生存期: Disease Free Survival

  • 定义: 结局指标改变为 由特定疾病导致的死亡，这时候开始关心死亡的原因是否是由特定疾病导致的。如果不是特定疾病导致的则不计入结局指标。
  • 优点: 针对性的反应临床获益，DSS提升能够很好的反应特定疾病的临床获益，特定疾病导致的死亡减少或增加。
  • 缺点: 同样很明显，相比简单确认患者是否死亡，这时候我们需要明确 何为由疾病导致的死亡？有没有明确的标准？，这个问题需要一个专业的判断。患者的死因经常并不容易明确。

• 疾病进展时间 time to progress，TTP

• 治疗失败时间 time to treatment failure，TTF

• 客观缓解率 objective response rate，ORR

grade

细胞看起来和组织得越多异常，癌症的等级就越高。高等级的癌细胞往往更具侵略性。它们被称为低分化或未分化。

一些癌症具有自己的肿瘤分级系统。大多数使用标准的1-4等级量表。

1级：肿瘤细胞和组织看起来最像健康的细胞和组织。被认为是低度肿瘤。
2级：细胞和组织有些异常，被称为中度分化。这些是中度肿瘤。
3级：癌细胞和组织看起来非常异常。
4级：这些未分化的癌症具有最异常的外观细胞。这些是最高等级的，通常比低等级的肿瘤生长和扩散更快。

stage

等级描述癌细胞和组织的外观，而癌症的阶段则说明原发性肿瘤的大小以及癌症在患者体内的扩散程度。
有几种不同的系统。其中许多是针对特定种类的癌症而创建的。其他可以用来描述几种类型的癌症。

阶段0至阶段IV
阶段0用于尚未扩散且不被视为癌症的异常细胞，尽管它们将来可能会癌变。此阶段也称为“原位”。
第一阶段至第三阶段适用于尚未扩散至原发肿瘤部位或仅扩散至附近组织的癌症。分期数越高，肿瘤越大，扩散越多。
第四阶段癌症已经扩散到身体的远处。

TNM分期

TNM分期系统是基于肿瘤的范围，淋巴结播散情况，是否存在转移完成的。

原发性肿瘤（T）
TX：主要肿瘤无法测量。
T0：找不到主要肿瘤。
T（is）或原位T：肿瘤仍在正常腺体范围内，不能转移。
T1，T2，T3，T4：是指主要肿瘤的大小和/或程度。T之后的数字越高，肿瘤越大，或者它越多地生长到附近的组织中。T可以进一步划分以提供更多细节，例如T3a和T3b。

局部淋巴结（N）
淋巴液将免疫系统细胞转运到全身。淋巴结是小的豆形结构，有助于移动这种液体。癌症通常首先扩散到附近的淋巴结并通过附近的淋巴结扩散。
NX：无法测量附近淋巴结中的癌症。
N0：附近的淋巴结无癌。
N1，N2，N3：指包含癌症的淋巴结的数量和位置。N之后的数字越高，则包含癌症的淋巴结越多。

远处转移（M）
转移是癌症向身体其他部位的扩散。
MX：无法测量转移。
M0：癌症尚未扩散到身体的其他部位。
M1：癌症已经扩散到身体的其他部位。
